This installment contract is to remain in effect for 12 full payments or until Beach Bumz receives a written and signed cancellation notice via certified U.S. Mail to:

- The initiation and/or membership fees and package prepayments are NOT refundable or transferable.
- All fees are paid in advance and due on the 1st business day of each month for a period of 12 months.
- All payments/fees will be debited monthly without notice from Beach Bumz Tanning.
- Beach Bumz Tanning reserves the right to cancel my membership at any time. We reserve the right to assign this authorization without notice.
- Beach Bumz Tanning may be closed during certain legal holidays and use of the membership is subject to interruption for needed repairs/maintenance.
- This contract is valid for 12 calendar months from the date below and will be charged whether services are used or not.
- All cancellation letters must be received at least 30 days before the scheduled billing date. Otherwise one final debit will be made as scheduled.
- You have the convenience of freezing (no tanning) your account for up to 3 months out of the year. There is a $5.00 fee for this convenience. Please refer to Freeze Form for details and/or ask to fill out a Freeze Form.
- A $50.00 cancellation fee will be charged if you cancel or default prior to 3 months from membership date.
- All chargebacks will add a $35.00 charge payable at the following scheduled debit.
- All NSF transactions will result in a $20.00 NSF fee unless other arrangements made.
- Any prior balance from previous month will be charged with your next scheduled deduction unless other agreements are made.
